UCFA is a company designed to provide healthcare practice services to individuals and families in the United States. UCFA grew from a concern that many families and individuals in our community are no longer able to get to a provider’s office and thus are not receiving good or even adequate healthcare. Many individuals are physically, emotionally, or mentally unable to deal with the stress of leaving their home. In many situations, there are no friends or family available to help.

 

UCFA provides in-home or remote healthcare visits. No matter where you are UCFA is close by. Our services range from face to face in our clinics, telehealth services, and remote offsite visits at work or home.

 

UCFA providers and staff are MD's, DO's, Nurse Practitioners, Physician Assistants, CCT Paramedics, Paramedics, BSN's, RN's, LPN's, Advanced EMT's, AAMA Medical Assistants, and other clinical support specialists.

 

We will visit with you routinely, to assist in providing the level of care that you need. If all is stable, we may visit once every 6-8 weeks. If there are any urgent needs or changes in your condition, we are on call 24/7 365 days with a less than 10 min response time to any home or facility within city limits.

 

We are not an emergency service but we do partner with local EMS for emergency transport with the ability to do direct admit for patients with primary care physician approval. Our goal is to help keep you stable and out of the hospital and the emergency department.
